When you are ready to enroll in a course or courses, select the
course or combination of courses you desire on the enrollment forms listed below.
When you
make your selection below, you will be linked to an enrollment form for your course.
You will be presented with several shipping price options depending on your area or region
of the world.
If you live outside the United States but have a US mailbox or shipping point, you will
then be able to take advantage of the US options, as the distinction between US/Canadian
and foreign enrollments have to do with the shipping and handling charges.
There are two portions to an enrollment form for you to fill out and submit. When
you submit your form, it is e-mailed to our offices in Tucson, Arizona (AZ) You
should fill out an enrollment for for each area of study you enroll in!
The first portion concerns your specific course or courses, the payment options and
shipping charges as stated on the page. The second portion consists of your
complete name, shipping address, phone numbers, email address as well as
data relating to your musical background and level of musical education. Be sure and
fill this page out to the best of your ability. If you are just starting out and
have little or no training, please state that information. This is not a
test, it is a background we need to better serve and help you.
If paying by credit card, you can enter it on the enrollment form.
However, if you prefer not to send your card number and expiration date via email, you can
fax it to us simply by printing out the enrollment form and FAXing it to (541)
552-9610. You can call us and we can in effect, enroll you over the phone.
